Check-In

 First, download the Check-In form.

 Complete the form and email it to VT-4 Sponsor Program.

The information will aid us in speeding up your check-in process, ordering your new nametags and coffee mug and it will also assist your sponsor and the Squadron in communicating with you.

NOTE: Coffee mugs are optional and all expenses will be billed to the Officer's mess buy-in on arrival.
